  • Step 2. Print out and complete this ORDER FORM.
  • Step 3. Visit an Audiologist or Hearing Aid Center and have full ear impressions made. (We will deduct the cost of the impression from your order).
  • Step 4. Securely ship ear impressions, order form and Audiologist receipt to:
